Cette substance contenait un acide uronique et un sucre amin\u00e9, elle a donc \u00e9t\u00e9 baptis\u00e9 acide hyaluronique (AH). <\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong>Durant les ann\u00e9es qui suivirent, l\u2019AH fut isol\u00e9 \u00e0 partir de nombreuses sources : l\u2019humeur vitr\u00e9e, le cordon ombilical humain ainsi que la cr\u00eate de coq (Fuoss et al, 1948). <\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong>En 1937, Kendall publia un article qui d\u00e9crivait la pr\u00e9sence d\u2019AH dans la capsule des bact\u00e9ries Streptococci de groupes A et C (Kendall et al, 1937). <\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong>Ce travail allait r\u00e9volutionner l\u2019utilisation de l\u2019acide hyaluronique bien des ann\u00e9es plus tard puisque la fermentation bact\u00e9rienne constitue encore actuellement la source d\u2019AH la plus fiable et la plus s\u00fbre dont nous disposons pour le d\u00e9veloppement des applications m\u00e9dicales.<\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong> Durant la m\u00eame p\u00e9riode, d\u2019autres observation ont permis de mettre en \u00e9vidence l\u2019existence d\u2019une mol\u00e9cule capable de d\u2019augmenter drastiquement la perm\u00e9abilit\u00e9 des tissus conjonctifs en provoquant la d\u00e9polym\u00e9risation de l\u2019AH. (Duran-Reynalds, 1929). <\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong>Ainsi, l\u2019\u00e9tude et l\u2019utilisation des hyaluronidases a permis de mieux comprendre la structure primaire de l\u2019AH. Parmi ceux-ci, l\u2019AH est le seul GAG non associ\u00e9 de mani\u00e8re covalente \u00e0 une prot\u00e9ine, le seul non synth\u00e9tis\u00e9 via l\u2019appareil de Golgi, et le seul qui n\u2019est pas sulfat\u00e9. Dans la peau sp\u00e9cifiquement, la quantit\u00e9 d\u2019AH mesur\u00e9e s\u2019\u00e9tablit \u00e0 0,5 mg\/g dans le derme et 100 mg\/g dans l\u2019\u00e9piderme (Reed et al, 1988) avec une demi-vie de 1,5 jour (Tammi et al, 1991).\u00a0<\/strong><\/span><\/h5><h5 class=\"p2\"><span style=\"color: #800080;\"><strong>Il est synth\u00e9tis\u00e9 par les \u00ab hyaluronan synthases \u00bb qui forment un groupe de 3 isozymes (HAS-1, HAS-2 et HAS-3). Durant l\u2019\u00e9volution des esp\u00e8ces, la structure de l\u2019acide hyaluronique n\u2019a pas chang\u00e9. L\u2019AH est principalement localis\u00e9 dans la zone fibreuse sous-cutan\u00e9e de cet organe. <\/strong><\/span><\/h5><h5 class=\"p3\"><span style=\"color: #800080;\"><strong>Avant l\u2019extraction, les cr\u00eates devaient \u00eatre lav\u00e9es \u00e0 l\u2019eau et \u00e0 l\u2019ac\u00e9tone, jusqu\u2019\u00e0 ce que la solution devienne transparente, pour \u00e9viter une d\u00e9gradation enzymatique et une oxydation d\u00e9l\u00e9t\u00e8re pour les mol\u00e9cules d\u2019AH (Swann DA, 1968). <\/strong><\/span><\/h5><h5 class=\"p3\"><span style=\"color: #800080;\"><strong>Puis, les tissus \u00e9taient broy\u00e9s pour entrer dans la phase d\u2019extraction proprement dite. <\/strong><\/span><\/h5><h5 class=\"p3\"><span style=\"color: #800080;\"><strong>Diff\u00e9rents solvants ont \u00e9t\u00e9 utilis\u00e9s comme l\u2019eau distill\u00e9e \u00e0 haute temp\u00e9rature, des solutions salines ou bien encore des m\u00e9langes contenant des solvants organiques.<\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>Des rendements de l\u2019ordre de 93 % ont pu \u00eatre obtenus lorsque la temp\u00e9rature du milieu \u00e9tait amen\u00e9e aux alentours de 80 degr\u00e9s Celsius. <\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>Ces conditions avaient l\u2019avantage d\u2019inactiver les hyaluronidases tissulaires, mais induisaient dans le m\u00eame temps une d\u00e9polym\u00e9risation partielle de l\u2019AH puisque cette mol\u00e9cule est sensible \u00e0 la temp\u00e9rature. <\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>Il convenait donc de trouver le bon compromis pour obtenir des tailles d\u2019AH compatibles avec les applications envisag\u00e9es.<\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>Apr\u00e8s l\u2019extraction, la phase de purification \u00e9tait \u00e9videmment extr\u00eamement importante pour s\u2019assurer d\u2019une biocompabilit\u00e9 maximale. <\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>En effet, des prot\u00e9ines, peptides, acides nucl\u00e9iques, lipides et mucoplysaccharides \u00e9taient pr\u00e9sents dans l\u2019extrait. <\/strong><\/span><\/h5><h5 class=\"p4\"><span style=\"color: #800080;\"><strong>Bien que les m\u00e9thodes utilis\u00e9es \u00e9taient essentiellement bas\u00e9es sur une s\u00e9rie de pr\u00e9cipitations (alcool) \/ dissolutions (eau), le fait de travailler \u00e0 partir d\u2019une source animale n\u00e9cessitait en compl\u00e9ment des techniques plus complexes telles que des hydrolyses enzymatiques ou l\u2019utilisation de solvants organiques comme le chloroforme.Akasaka et al, 1985 ; Notons aussi que plus <span class=\"s1\">collagen fibers <\/span>sont organis\u00e9es et align\u00e9es, plus la valeur de tension maximale au-del\u00e0 de laquelle les fibres sont irr\u00e9m\u00e9diablement endommag\u00e9es est grande (Aziz J et al, 2016).\u00a0<\/strong><\/span><\/h5><h5 class=\"p1\"><span style=\"color: #800080;\"><strong><span class=\"s1\">Elastic fibers <\/span>sont un groupe de prot\u00e9ines matricielles qui sont synth\u00e9tis\u00e9es par le fibroblaste par un processus complexe appel\u00e9 \u00e9lastog\u00e9n\u00e8se bas\u00e9e sur un pr\u00e9curseur, la tropo\u00e9lastine. De la m\u00eame fa\u00e7on que le collag\u00e8ne est indispensable pour donner \u00e0 la peau sa r\u00e9sistance \u00e0 la traction, <span class=\"s1\">elastin <\/span>gives it the property to expand and then return to normal without suffering irreversible mechanical damage (Rauscher S et al, 2012).\u00a0<\/strong><\/span><\/h5><h5 class=\"p1\"><span style=\"color: #800080;\"><strong>Ce type de fibre est abondant dans le derme, et son importance est illustr\u00e9e lorsque l\u2019on \u00e9tudie par exemple les sympt\u00f4mes des patients atteints de cutis laxa (maladie rare qui touche le syst\u00e8me d\u2019assemblage des fibres \u00e9lastiques). Outre des atteintes graves au niveau des syst\u00e8mes respiratoires, digestifs et vasculaires, la peau de ces patients apparait molle, sans aucune tenue et pr\u00e9sente pr\u00e9matur\u00e9ment un aspect \u00e2g\u00e9 (Andiran N et al, 2002).\u00a0<\/strong><\/span><\/h5><h5 class=\"p1\"><span style=\"color: #800080;\"><strong>Ces donn\u00e9es illustrent donc le r\u00f4le pr\u00e9pond\u00e9rant tenu par le r\u00e9seau de fibres d\u2019\u00e9lastine dans les propri\u00e9t\u00e9s biom\u00e9caniques de la peau. <span class=\"s1\">Collag\u00e8nes et \u00e9lastine repr\u00e9sentent donc les \u00e9l\u00e9ments principaux de la matrice extracellulaire du derme. Elle peut par cons\u00e9quent \u00eatre qualifi\u00e9e d\u2019extr\u00eamement dense <span style=\"color: #0000ff;\">(comme illustr\u00e9 dans la Figure 7).<\/span>\u00a0<\/span><\/strong><\/span><\/h5><h5 class=\"p1\"><span style=\"color: #800080;\"><strong><span class=\"s1\">Ainsi, un gel d\u2019acide hyaluronique injectable d\u00e9di\u00e9 \u00e0 ce plan d\u2019injection devra n\u00e9cessairement \u00eatre adapt\u00e9 rh\u00e9ologiquement \u00e0 cet environnement particulier. Compar\u00e9 au derme, tr\u00e8s peu d\u2019auteurs se sont consacr\u00e9s \u00e0 la biom\u00e9canique du tissu adipeux, mais un certain nombre de travaux ont montr\u00e9 que le module de Young tissus graisseux pouvait varierde 0,5 \u00e0 25 KPa (Kilo Pascal) (Gefen A et al, 2007).\u00a0<\/strong><\/span><\/h5><h5 class=\"p3\"><span style=\"color: #800080;\"><strong>Il est int\u00e9ressant de comparer ces valeurs aux 21 \u00e0 39 -MPa (Mega Pascal) mesur\u00e9s sur le derme (McKee et al, 2011) puisqu\u2019elles sont tr\u00e8s nettement inf\u00e9rieures. La coh\u00e9sivit\u00e9 est donc un facteur important \u00e0 consid\u00e9rer en association avec la viscosit\u00e9 quant au respect et \u00e0 la pr\u00e9servation des structures intimes de la matrice extracellulaire (Tran C et al, 2014). Indeed, we have seen above that endogenous hyaluronic acid is a very fragile molecule with a low half-life. To increase its life span, the cross-linking reaction allows the HA chains to be brought closer together. To do this, it is necessary to use a reactive molecule, capable of creating covalent bonds between 2 HA molecules.
