It is thus considered as a <\/span><i><span style=\"font-weight: 400;\">alternative cellular product<\/span><\/i><span style=\"font-weight: 400;\"> allowing to avoid the culture and cell expansion steps of MSCs.<\/span><\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"color: #000000;\"><span style=\"font-weight: 400;\">Unfortunately, in France, the use of MSCs and SVF must meet regulatory constraints, being <\/span><i><span style=\"font-weight: 400;\">products of cell therapy<\/span><\/i><span style=\"font-weight: 400;\"> or <\/span><i><span style=\"font-weight: 400;\">\"Innovative Therapy Medicines,\" ITM<\/span><\/i><span style=\"font-weight: 400;\">\u00a0\"(European Directive 2009\/12\/EC), thus limiting their use by research teams for clinical validation.\u00a0<\/span><\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"color: #000000;\"><span style=\"font-weight: 400;\">These limitations have led to <\/span><i><span style=\"font-weight: 400;\">develop new techniques for transferring adipose tissue without going through digestion or cell culture steps<\/span><\/i><span style=\"font-weight: 400;\">. Thus, <\/span><i><span style=\"font-weight: 400;\">the development of cannulas<\/span><\/i><span style=\"font-weight: 400;\"> allowing the collection of <\/span><i><span style=\"font-weight: 400;\">\"micro\" adipose lobules<\/span><\/i><span style=\"font-weight: 400;\"> has allowed the use of a fat rich in stem cells.\u00a0<\/span><\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"color: #000000;\"><span style=\"font-weight: 400;\">More recently, <\/span><i><span style=\"font-weight: 400;\">Tonnard et al. <\/span><\/i><span style=\"font-weight: 400;\">have described a <\/span><i><span style=\"font-weight: 400;\">mechanical technique<\/span><\/i><span style=\"font-weight: 400;\"> allowing to obtain <\/span><i><span style=\"font-weight: 400;\">emulsified fat or \"nanofat<\/span><\/i><span style=\"font-weight: 400;\"> from a sample of <\/span><i><span style=\"font-weight: 400;\">microfat<\/span><\/i><span style=\"font-weight: 400;\">. <\/span><i><span style=\"font-weight: 400;\">According to the authors, this emulsified fat would be rich in progenitor cells.<\/span><\/i><\/span><\/li><\/ul><p><span style=\"color: #000000;\"><i><span style=\"font-weight: 400;\">These regenerative medicine tools should be used and validated for specific indications in each surgical field.<\/span><\/i><\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-0e43bc9 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"0e43bc9\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-e8b3176\" data-id=\"e8b3176\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-811b886 elementor-widget elementor-widget-heading\" data-id=\"811b886\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">I\/ Lipofilling or fat grafting: an advanced therapy\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c42451e elementor-widget elementor-widget-text-editor\" data-id=\"c42451e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"font-weight: 400; color: #0000ff;\">For a long time, lipofilling was considered to be \"volumizing\" but the discovery of its trophic character (2000) made lipofilling the most widely used regenerative medicine treatment.<\/span><\/p><p><span style=\"font-weight: 400;\">Recent studies have shown that <\/span><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">stroma-vascular fraction of adipose tissue<\/span><\/i><\/span><span style=\"font-weight: 400;\"> represented <\/span><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">a reservoir of precursor cells with pro-angiogenic potential<\/span><\/i><\/span><span style=\"font-weight: 400;\"> was comparable to that of bone marrow-derived stem cells (1).\u00a0<\/span><\/p><p><span style=\"font-weight: 400;\">In addition, it has been shown that <\/span><i><span style=\"font-weight: 400;\"><span style=\"color: #0000ff;\">mesenchymal stem cells<\/span> <span style=\"color: #0000ff;\">(CSM)<\/span><\/span><\/i><span style=\"font-weight: 400;\"> were also present in the adipose tissue.\u00a0<\/span><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">The latter therefore represents a new potential reservoir of pluripotent cells that could be used in regenerative medicine<\/span><\/i><\/span><span style=\"font-weight: 400;\">.<\/span><\/p><p><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">Autologous fat transfer<\/span><\/i><\/span><span style=\"font-weight: 400;\"> is a procedure already applied to obtain the increase of soft tissue volume losses.\u00a0<\/span><span style=\"font-weight: 400;\">The fat grafts removed by liposuction are reinjected subcutaneously to restore volume to the defective areas.\u00a0<\/span><\/p><p><span style=\"font-weight: 400;\">Thus, <\/span><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">adipose tissue transplantation<\/span><\/i><\/span><span style=\"font-weight: 400;\"> is used to correct congenital deformities and complex traumatic wounds with loss of soft tissue, after oncological surgery, after sequelae of radiotherapy... (2).<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-1425f4c elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"1425f4c\" data-element_type=\"section\" data-e-type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-f387c31\" data-id=\"f387c31\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-5c1e837 elementor-widget elementor-widget-heading\" data-id=\"5c1e837\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">II\/ New techniques for transferring adipose tissue rich in progenitor cells\n\n<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-1826a36 elementor-widget elementor-widget-text-editor\" data-id=\"1826a36\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">One of the constraints of the use of ITMs in the clinic is the time required and the costs associated with their in vitro amplification. <\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">These limitations have led to the development of new cellular products.<\/span><\/li><\/ul><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><em><span style=\"font-weight: 400;\">Several techniques have been proposed for the harvesting of adipose tissue. <\/span><\/em><i><span style=\"font-weight: 400;\">Coleman et al (3)<\/span><\/i><span style=\"font-weight: 400;\"> described a technique that minimizes trauma to the adipocytes. Using a 2-hole, 3-mm cannula with rounded edges connected to a 10-ml syringe, fat is manually aspirated by removing the plunger.\u00a0<\/span><span style=\"font-weight: 400;\">The cannula is pushed through the harvest site, the surgeon pulls the plunger of the syringe and creates a slight negative pressure that allows flaps of fat to pass through the cannula and the Luer-Lock opening in the syringe barrel.\u00a0<\/span><span style=\"font-weight: 400;\">Once filled, the syringe is disconnected from the cannula, which is replaced with a cap that seals the Luer-Lock end of the syringe.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><em><span style=\"font-weight: 400;\">The grease drawn into the syringes is centrifuged at 3000 RPM (revolutions per minute) for 3 minutes to isolate the grease.\u00a0<\/span><\/em><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">However, reducing the centrifugation time to 1 min has recently been shown to be of value for cell viability (4).<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><em><span style=\"font-weight: 400;\">After centrifugation, three layers are observed:\u00a0<\/span><\/em><ul><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">The first layer includes oil, which can be removed with an absorbent material,\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><em><span style=\"font-weight: 400;\">The second layer is made up of adipose tissue,\u00a0\u00a0<\/span><\/em><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">And the third layer contains blood, tissue fluid and local anesthetic and is ejected from the base of the syringe.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"2\"><em><span style=\"font-weight: 400;\">The middle layer is used for fat grafting (5).<\/span><\/em><\/li><\/ul><\/li><\/ul><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><em><span style=\"font-weight: 400;\">New techniques for fat harvesting have been developed using cannulas of only 0.7mm in diameter, used to treat delicate areas of the face such as the eyelids and lips.<\/span><\/em><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-3df08bb elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"3df08bb\" style=\"font-weight: 400;\"> \u00a0<\/span><\/span>Macrofat fat is more structural in nature and is easily injected through an 18G or 19G (gauge) cannula.\u00a0<\/p><p><span style=\"color: #0000ff;\"><i><span style=\"font-weight: 400;\">Macrofat fat is used for structural augmentation:<\/span><\/i><\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">FSV is isolated by digesting the lipid portion of the lipoaspirate with collagenase, separating the contents into two distinct phases: the floating fraction of mature adipocytes and the cellular components of interest in the lower water fraction (24-25).\u00a0<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">This separation can be improved by centrifugation, but nevertheless a comparable separation can be achieved by phase separation and gravity filtration (26).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Centrifugation is more efficient because it also allows sedimentation of all cells present, whereas filtration can be designed to capture only important cell types based on their size, thus enriching the specific cell cocktail.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Centrifugation of the aqueous fraction yields a reddish pellet that contains FSV cells.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Erythrocytes, a major contaminant present in the SVF pellet, can be lysed to isolate a purer population of stem cells and\/or SVF cells if they are intended for in vitro expansion (27).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The VSF contains a variety of cells: mesenchymal stem cells, pericytes, vascular cells, fibroblasts, pre-adipocytes, monocytes, macrophages, red blood cells, fibrous tissue and extracellular matrix (ECM).\u00a0<\/span><\/li><\/ul><p><i><span style=\"font-weight: 400;\">For clinical applications in regenerative medicine, one of the advantages of using FSV is its extemporaneous use without going through the cell culture steps.<\/span><\/i><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The number of stem cells contained in adipose SVF can fluctuate considerably as patients may have different adipose tissue texture and density cells<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-515ead2 elementor-widget elementor-widget-text-editor\" data-id=\"515ead2\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><i><span style=\"font-weight: 400;\">Adipose Derived Stem Cells (ADSC)<\/span><\/i><span style=\"font-weight: 400;\"> were first characterized in 2001 and have since been widely studied and used as a major source of cells with regenerative potential, with characteristics similar to those of mesenchymal stem cells (MSCs) (29-31).\u00a0<\/span><span style=\"font-weight: 400;\">Stem cells are cells capable of self-renewal, generating identical daughter cells to maintain the stem cell pool, and differentiating into multiple cell lines (progenitors with a more restricted potential).\u00a0<\/span><\/p><p><span style=\"color: #0000ff;\"><b>\u00a0 \u00a0 \u00a0 \u00a0 Characterization of adipose tissue MSCs (differentiation potential, proliferation, paracrine and immunomodulatory actions)<\/b><\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Several studies have demonstrated the ability of adipose tissue stem cells to differentiate into adipogenic, osteogenic and chondrogenic lineages, and even into myogenic lineages, leading to skeletal muscle, smooth muscle and cardiomyocytes (32).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">MSCs have also been shown to possess the potential to differentiate into cells of both ectodermal and endodermal lineages, such as neuronal cells, endothelial cells, epithelial cells, hepatocytes, pancreatic cells, and hematopoietic cells (33-39).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">In addition, they also have a migration potential (40) and a capacity to proliferate that can be stimulated by various growth factors. (41).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The main effects of MSCs are mediated by paracrine activity. ASCs produce and secrete a wide variety of growth factors, cytokines, and chemokines that may be involved in wound healing by secreting angiogenic and anti-apoptotic growth factors (42).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">In addition, adipose tissue MSCs modulate monocytes, macrophages, T and B lymphocytes by inducing the host immune response.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b style=\"color: #0000ff; font-family: var( --e-global-typography-text-font-family ), Sans-serif;\">MSCs from adipose tissue in cell therapy<\/b><\/li><\/ul><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">One of the advantages of using MSCs from adipose tissue, and not the least, is the ease of obtaining these cells. They do not pose any ethical problems, unlike embryonic stem cells.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indeed, they are recovered directly from the fat of the \"surgical waste\", after dermolipectomy or liposuction. 10 to 100 million stem cells can be obtained from 300 milliliters of lipoaspirate of which more than 90% would be viable (43). A large number of stem cells can be obtained in a few passages.<\/span><\/li><\/ul><ul><li aria-level=\"1\"><span style=\"color: #0000ff;\"><b>Optimization of cell therapy protocols using adipose tissue MSCs<\/b><\/span><\/li><\/ul><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Regenerative medicine, which aims to stimulate tissue repair and regeneration mechanisms, offers the possibility of effective treatment strategies in combination.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">However, in this field, the tissue engineering approaches proposed so far have involved in vitro expansion of autologous or allogeneic cells (44).\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The future of reconstructive surgery will favor autologous cell therapy, if possible extemporaneous, for skin regeneration.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Despite promising results in animal models of injury, the use of ASCs (<\/span><i><span style=\"font-weight: 400;\">Adipose Stem Cells)\u00a0<\/span><\/i><span style=\"font-weight: 400;\"> has shown mixed results in clinical trials for skin healing (45), due to their poor viability after transplantation.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\">In a study published in \"International Stem Cells\", we showed that the combination of PRP with cell therapy by injection of adipose tissue MSCs seems to improve skin healing in the mouse model.\u00a0<\/li><\/ul><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indeed, treatment with MSC injections and the combination of MSC and PRP significantly improves skin regeneration.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Time to healing was also significantly improved by the combination of MSC and PRP compared to MSC alone, PRP alone and the control.\u00a0<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">We were able to explain these preclinical results by the demonstration of activation of the MSC secretome by PRP, reflected by activation of human-derived VEGF and IL-6 transcriptional expression in mouse transplant biopsies and also by increased MSC survival (46).<\/span><\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section of the intervention (therapeutic or aesthetic) that determines the applicable regulations.\u00a0<\/strong><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><strong><span style=\"color: #ff0000;\">It is therefore possible to use PRP in functional or reconstructive surgery (e.g. scarring).<\/span><\/strong><b style=\"color: var( --e-global-color-text ); font-family: var(
